Hey Nitin - appreciate your feedback. I have a couple other variations of the MoM forms, one of which accomplishes what you outlined. Also have Issue Logs and Risk Logs in Excel which are along the same lines. I've leaned towards the uploaded version of the MoM forms because they have fit my projects, teams and leadership style better. But I'll share the other variations in the near future.

I am keen to hear your thoughts on showing other health parameters on the dashboard - where/how would you suggest incorporating some other elements - as icons/indicators, as color coded sub-charts over the process, etc. Look forward to hearing from you.

I appreciate the forms you have created.
Few suggestions:
MoM forms need to be more objective type & hence to be in Excel format (with date of point of discussion opened & closed),responsible person, status (open/closed) etc.
Dashboard needs to have many other health parameters including Project & Product Cost, status of design, service, launch, risks & so on.
